Please enclose your valid passport.

Your U.S. passport should be valid for at least 6 months beyond the date you intend to enter into Brazil.

Your passport should have one completely blank page for your visa. (Please note that pages 22-24 should not be considered for Visas. The Brazilian consulate will not place a visa on these pages.)

Print ea. page of the 4 page application and the receipt,with Protocol #,sign and send to us with all other documents.
Make sure your passport is signed.
Please include $160.00 ( $220.00 if business) POSTAL money order for Brazilian Consulate.For each visa requested. If we have to purchase money order there is an additional $5.00 charge ea.
Balance of fees ,$80.00 ea.applicant, plus the FedEx fees may be made by check, money order,Visa, or Mastercard,to Adams Passports and Visas. .

SUPPORT
Support is documentation confirming your trip and the reasons for your visit. Support is slightly different if you are traveling for Business or as a Tourist. We will need copies of the following:
TOURIST –
- A photocopy of your flight itinerary.
- A copy of your Driver’s License
- If you are under 18 years of age, you must submit copy of Birth Certificate.
FOR TRAVELERS UNDER 18 WHO ARE TRAVELING ALONE OR WITH ONE PARENT –
A notarized letter from the non-traveling parent or parents giving permission for the child
to travel to Brazil.
An ORIGINAL birth certificate for the child traveler.
BUSINESS –
- A photocopy of a round-trip ticket or a booked itinerary from a travel agent.
- A copy of your Driver’s License.
- Letter(on company's letterhead) addressed to the Consulate General of Brazil, stating the employee's name and titile, contact in Brazil, how long he/she has been working for the company, as well as the nature of business that he/she will conduct in Brazil. The company must state they will assume full responsibility for the applicant while in Brazil- Individuals involved in installation,service and/or repair of equipment in Brazil, do not qualify for a business visa.
- If applicant is self-employed, an additional letter will be required from his/her bank, containing financial references.
- An invitation from your host organization.
NON-U.S. PASSPORT HOLDERS -
- A copy of your green card, front and back, or a photocopy of your U.S. visa.
FOR TRAVELERS UNDER 18 YEARS OLD WHO ARE TRAVELING ALONE
OR WITH ONE PARENT -
- A notarized letter from the non-traveling parent or parents giving permission for the child to travel to brazil.
- An ORIGINAL birth certificate for the child traveler.
**IMPORTANT FOR ALL TRAVELERS** If you are traveling to Peru before you travel to Brazil, then you MUST submit proof of yellow fever vaccination in order to get your visa. The Brazil Consulate MUST see your vaccination certificate.
